Membership and pricing
One clear monthly fee covers your primary care, with cash-pay pricing and convenient access to Dr. Miya Yi, DNP, FNP-BC, your family nurse practitioner. There's no charge for visits, and we never bill your insurance.

Family maximum
Up to two parents and their children 17 and under
$350 a month at most
Everyone must live at the same address. Family members 18 and older join as individual members.
For example, two adults under 55 pay $238 a month, two adults and one child pay $297, and two adults with two or more children pay $350.
Enrollment fee
$100 per person, up to $250 per family. It covers setting up your medical record, reviewing your health history and medications, and preparing your personal care plan.
The first 50 members have the enrollment fee waived.
